HSE Home Support Services

The HSE Home Support Service is free and available to people who may need support to continue living at home or to return home following a hospital stay.

How the HSE Home Support Service Works in 6 Simple Steps:

You can complete an application form yourself or someone can apply on your behalf. This could be your relative, a family carer, GP, or Public Health Nurse. Application forms can be found on the HSE website, or your local Home Support Office can send you a form. If you are in hospital, you or your family should ask the person dealing with your discharge plan (usually the Medical Social Worker) for an application form. This should be done as soon as possible during your hospital stay to avoid any delays.

Not many people know that you can choose your preferred Care Provider when filling in your application. There is a list of HSE approved providers on every application form and we would be very happy to have your first choice. You will find Finglas Home Care listed under “Dublin Home Care Partners Consortium” – this is a group of five other Irish non-profit organisations with whom we work closely and who share the same passion and values to provide high quality home care in Dublin.

The HSE will arrange an assessment to determine your home support needs. This assessment is most often conducted by your Public Health Nurse. They will consider your ability to carry out everyday tasks, your family and social supports, and your wishes and preferences. 

 The HSE will write to you to let you know what supports can be provided. We are an approved HSE service provider so as soon as we receive your HSE Home Support Service referral, your Care Coordinator will contact you to introduce themselves.

Your Care Coordinator will go through your finalised home support plan and complete a home risk assessment. They will listen to your needs and answer any further questions that you might have.

On the day your service begins, your Care Coordinator will introduce your Carer. Your Care Coordinator will also keep in regular contact with you to ensure you are happy with the service we are providing. 

Private Care

We understand that home support can be short or long term; often from as little as a few days extra care to help a loved one get back on their feet or regain their confidence at home, to several weeks/months or longer.

We also recognise that you may simply wish to supplement a HSE Home Support Service with a little extra support when needed. Our rates are very competitive, and you may also be eligible for Tax relief.

How Our Private Home Support Service Works in Five Simple Steps:

To book your consultation, all you have to do is call us on 01 864 0350 and request a consultation for private home care.

A Care Coordinator or Manager will call you and conduct a free consultation. They will listen to your needs, offer advice on your home support options, discuss your care plan, and answer any queries you may have.

Once you have chosen a home support plan and are happy to proceed, we will make all of the arrangements. The Care Coordinator will identify the Carers that best suit your needs.

The Care Coordinator will call to complete your registration for our home support services. Your Care Coordinator will go through your finalised home support plan and complete a home risk assessment, as well as answering any further questions that you might have.

On the day your service begins, your Care Coordinator will introduce your Carer. Your Care Coordinator will also keep in regular contact with you to ensure you are happy with the service we are providing.
